Please fill in your name, address, and telephone number,
how many of each item you want to purchase and, for each group of items,
the area for postage (U.K./Europe/Elsewhere), putting in the number of
items rather than the amount of postage. In the price section of the form
you just need to fill in the number of each item you want to buy [not the cost]
and, when you click in another box, the total will be automatically updated.
Finish by clicking in the Total box to update the total with the final item or
postage. Add how many of each size are required for the sweatshirt and polo
shirt, print out the form and send with your cheque/postal order (only in pounds
